Area: Stark County & Surrounding Areas (Dover, New Philadelphia, Carrollton, and surrounding communities)
Monday–Friday 8-5 pm

As a Territory LPN, you'll work independently while being supported by an experienced clinical team. You'll build meaningful relationships with patients and families, make a direct impact on patient outcomes, and enjoy variety in your day-to-day work.

Position Details
Monday–Friday schedule
Average of 6 patient visits per day
Territory Nurse role covering Stark County and portions of Tuscarawas and Carroll Counties
Daily visits are scheduled geographically to maximize efficiency whenever possible
Participation in agency on-call and weekend rotation required
Two holidays per year required
What You'll Do
Provide skilled nursing care in patients' homes under physician orders and RN direction
Assess and monitor patient conditions
Administer medications and treatments
Educate patients and families on disease management and preventative care
Collaborate with physicians, case managers, and interdisciplinary team members
Complete timely clinical documentation in accordance with agency and Medicare guidelines
Help reduce hospital readmissions through patient education and proactive care
Qualifications
Current Licensed Practical Nurse (LPN) license
Minimum one year of acute care experience required
Home health experience preferred
Strong assessment, communication, and critical thinking skills
Valid driver's license and active auto insurance
Ability to travel throughout the assigned territory
