- 8-hour day shifts
- No weekends
- Hybrid position
- Provides support to Cambridge, Coon Rapids and Forest Lake clinics, with regular onsite presence at each location approximately 2-4 times per month
Job Description:
Facilitate, empower, and enable providers and practice staff to achieve success in a patient-centric and clinically driven value-based care model. Drive cost-efficiency, quality, access, and overall patient experience through supporting the physician and patient relationship.
Principle Responsibilities
- Builds and maintains relationships with practice operational and clinical leadership
- Informs the development of performance improvement initiatives to achieve identified goals.
- Impact the Quadruple Aim by improving physician satisfaction across the areas of personal, professional and income lifestyle to decrease physician burnout.
- Serves as a subject matter expert in value-based care
- Serves as a thought partner and a trusted point of contact.
- Identifies educational needs and proactively seeks to address these barriers across the practice team.
- Develops and deploys localized practice optimization strategies
- Implement strategies based on data analysis that includes measurable and time-specific aim statements.
- Utilize data-informed Plan-Do-Study-Act rapid cycle optimization approaches.
- Analyze and interpret trends using internal and external data.
- Implement proactive strategies to address issues and measure Practice Optimization Program impact.
- Holds practice optimization meetings regularly with practice leadership and key clinical influencers to monitor progress of deployed practice optimization strategies.
- Share best practices, provider feedback, and opportunities for organizational alignment.
- Other duties as assigned.
Required Qualifications
- Bachelor’s degree in Business or Healthcare related field or equivalent experience
- 5+ years of healthcare leadership experience. Demonstrated ability to use data and lead operational change, with experience using the Plan-Do-Study-Act rapid cycle optimization to influence utilization, cost, and quality metrics
Preferred Qualifications
- HCC/RAF knowledge
- HEDIS/Quality knowledge
- Primary Care experience strongly preferred

Pay Range
Pay Range: $42.92 to $59.38 per hourThe pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience and education. Additional incentives may be available, including discretionary monetary bonuses, goal-based bonuses, and other financial incentives.Benefit Summary
