Case Manager - Parents Plus

This position provides home and community-based services for DCFS youth that are pregnant and/or parenting. This position provides a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to meet the youth's comprehensive needs through coordination and coaching.
