Admissions (Care) Coordinator- Stone Oak Care Center

What You’ll Do: Deliver the Touchstone Experience

Be the First Impression
Serve as the welcoming voice and face of our community, ensuring every resident and family feels valued, supported, and at ease from day one.
Build Lasting Relationships
Cultivate strong partnerships with hospitals, physicians, and discharge planners—becoming a trusted resource for seamless transitions into care.
Guide with Compassion & Confidence
Walk families through the admissions journey with empathy, clarity, and professionalism, helping them feel informed and reassured every step of the way.
Coordinate with Excellence
Evaluate referrals, ensure clinical alignment, and facilitate efficient, timely admissions while maintaining accuracy in all documentation and eligibility processes.
Create a Seamless Arrival Experience
Collaborate with interdisciplinary teams to deliver a smooth, personalized move-in experience that reflects our commitment to quality and care.
Drive Impact
Track leads, manage patient-served (census) data, and contribute to occupancy goals—all while keeping the resident experience at the heart of every decision.

What You Bring

  • Must have at least one year of previous sales/admissions/marketing experience in the skilled nursing industry. 
  • A natural ability to connect, influence, and build trust with diverse audiences
  • Strong organizational skills with a keen eye for detail
  • Confidence navigating EMRs and referral tracking tools
  • A results-driven mindset balanced with genuine compassion and service
