Clinical Navigation Specialist, Oncology - Cancer Services - IMC

This position will provide individualized assistance to patients, families, and caregivers throughout their Cancer journey from diagnosis through survivorship. Works directly with our Oncology providers and serve as a consistent care coordinator and patient advocate, ensuring timely access and seamless transition across the oncology care continuum. Responsible to facilitate rapid scheduling, coordinate care across service lines and sites of care, identify and remove barriers to care, track patients to ensure compliance with treatment and surveillance plan. Provides patient and family education related to disease processes, as well as available internal and external support services and resources. Serves as a clinical expert in facilitating and delegating all care aspects. Participates in our accreditations and ensure that compliance is met with our accreditation standards. Services provided will be billable in conjunction with the lead provider.
