Job Description

The Registered Nurse (RN) is a key member of the Assertive Community Treatment (ACT) / ACT Plus multidisciplinary team and is responsible for providing comprehensive nursing care and clinical support to individuals with severe and persistent mental illness, co-occurring disorders, and complex healthcare needs. The RN delivers community-based nursing services that support recovery, rehabilitation, health maintenance, medication management, and independent living.

Working collaboratively with behavioral health professionals, medical providers, families, and community partners, the Registered Nurse promotes holistic, person-centered care that enhances participant health, stability, recovery, and quality of life.


RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS

Direct Duties

Clinical Assessment & Nursing Care

  • Conducts comprehensive psychiatric, clinical, and physical health assessments.
  • Evaluates participant health needs and develops nursing interventions to support treatment goals.
  • Monitors participant physical and behavioral health status.
  • Provides direct nursing care and health-related services in community and office-based settings.
  • Assesses participants for changes in physical, psychiatric, and behavioral conditions.
  • Identifies actual or potential health concerns and communicates findings appropriately.
  • Maintains accurate clinical documentation and nursing records.

Medication Management

  • Administers and manages medications and prescribed treatments.
  • Provides medication education to participants and families.
  • Monitors medication effectiveness, side effects, and adherence.
  • Administers injections and other ordered treatments as prescribed.
  • Maintains medication inventories and ensures proper storage, handling, and documentation.
  • Ensures compliance with medication administration regulations and standards.
  • Coordinates prescription refills and medication-related services with pharmacies and providers.

Community-Based Services & Recovery Support

  • Provides community-based nursing and support services to participants.
  • Delivers in-home services including:
    • Medication training
    • Activities of Daily Living (ADL) training
    • Home management support
    • Health education
  • Supports participants in achieving recovery, wellness, and independent living goals.
  • Promotes self-management of health conditions and treatment adherence.
  • Assists participants in navigating healthcare systems and accessing services.

Health Education & Counseling

  • Educates participants regarding:
    • Mental health conditions
    • Physical health conditions
    • Medications
    • Disease prevention
    • Wellness practices
    • Recovery strategies
  • Provides supportive counseling and encouragement.
  • Assists participants in understanding treatment recommendations and healthcare plans.
  • Provides consultation and support to family members and caregivers.

Crisis Intervention & Care Coordination

  • Coordinates emergency and crisis intervention services when necessary.
  • Assesses urgent clinical situations and initiates appropriate interventions.
  • Participates in crisis prevention and response planning.
  • Coordinates medical and behavioral healthcare services.
  • Ensures follow-through on medical appointments, treatments, and provider recommendations.
  • Communicates treatment changes and significant clinical information to team members and providers.
  • Participates in on-call responsibilities as assigned.

Other Responsibilities

Interdisciplinary Team Collaboration

  • Participates in treatment planning meetings and interdisciplinary case conferences.
  • Collaborates with psychiatrists, therapists, case managers, peer specialists, and other providers.
  • Contributes nursing expertise to individualized treatment plans.
  • Supports person-centered, recovery-oriented service delivery.

Resource Coordination & Advocacy

  • Assists participants in accessing healthcare, housing, employment, and community resources.
  • Coordinates referrals and linkages to external healthcare and support services.
  • Advocates on behalf of participants to improve access to services and supports.
  • Supports participant rights, self-determination, and community integration.

Compliance & Quality Assurance

  • Ensures compliance with:
    • Nursing practice standards
    • Licensing requirements
    • Medication regulations
    • Agency policies
    • Behavioral health standards
  • Supports infection prevention and disease control activities.
  • Maintains relationships with:
    • Medical providers
    • Pharmacies
    • Hospitals
    • Ancillary healthcare services
  • Participates in quality improvement initiatives and program development.

Staff Training & Support

  • Provides consultation, support, and education to interdisciplinary team members.
  • Trains and supervises unlicensed staff regarding health-related procedures and care standards.
  • Promotes safe and effective healthcare practices throughout the program.

ADHERE TO AND FOLLOW

  • The By-Laws of the Corporation and as set forth in the Articles of Incorporation.

Job Qualifications

Education and Experience

Required

  • Graduate of an accredited School of Nursing.
  • Current unrestricted Registered Nurse (RN) license in the State of Delaware.
  • Successful completion of required background investigations and security clearances.
  • Pre-employment Tuberculin Skin Test (TB Screening).
  • CPR Certification.

Preferred

  • Bachelor of Science in Nursing (BSN).
  • Minimum one (1) year of nursing experience.
  • Experience working in:
    • Behavioral Health
    • Community Mental Health
    • Assertive Community Treatment (ACT)
    • Psychiatric Nursing
    • Substance Use Treatment
    • Integrated Healthcare Settings

Licenses & Certifications

  • Current Delaware Registered Nurse License.
  • CPR Certification.
  • Valid driver’s license with acceptable driving record.
  • Ability to maintain security clearance requirements, including Delaware Department of Correction approval where applicable.
