Social Worker | Care Manager (MHRT/C or Eligible) - Pediatrics

PCHC’s Care Management team is seeking a Social Worker/Care Manager to join our mission-driven, interdisciplinary care teams. In this role, you’ll partner with pediatric patients, caregivers, and providers to coordinate care and connect families with vital community resources that support healthy development and long-term well-being. This role is ideal for someone who enjoys working closely with families, navigating complex dynamics, and advocating for children across systems of care.
