We are seeking a highly skilled and empathetic Advanced Practice Registered Nurse (APRN) to join our telehealth team. This specialized role focuses on providing comprehensive memory assessment and care through a virtual platform. You will be responsible for conducting cognitive screenings managing the symptoms of patients living with dementia.A portion of this role involves managing the complex behavioral and psychological symptoms of dementia (BPSD), ensuring that both patients and their caregivers have a clear, medically sound path forward from the comfort of their homes.
How will you make an impact & Requirements
Key Responsibilities
Virtual Assessments: Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations.
Cognitive Screening: Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment.
Psychiatric Management: Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions.
Caregiver Support: Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques.
Documentation: Maintain electronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth.
Qualifications & Requirements
Licensure: Must hold a current, unencumbered Florida APRN license with prescriptive authority.
Education: Master’s or Doctoral degree in Nursing (MSN or DNP).
Clinical Experience: Minimum of 2–3 years of experience specifically in geriatrics, neurology, or a dedicated memory care setting preferred.
Telehealth Proficiency: Experience conducting video-based clinical visits and comfort troubleshooting basic technology issues with elderly patients and their families.
Specialized Knowledge: Working knowledge of the CMS Cognitive Assessment & Care Plan requirements and the nuances of dementia-related behavioral health.
