JOB TITLE: Post Acute Coordinator WKE

DEPT: Care Management

LOCATION: Maury Regional Medical Center

SHIFT: Weekend Sat-Sun 7a-7p

ABOUT THE UNIT:

The Post Acute Coordinator works directly with the care manager and nurse navigator in daily discharge planning functions. This includes assisting with the provision of patient choice for post-acute services and ensuring these choices and referrals are well documented in the EHR. The position assists in the delivery of required regulatory notices. 

JOB REQUIREMENTS:

  • LPN Tennessee Preferred
  • 2 Years Medical/Surgical Nursing Experience Preferred
  • 2 Years Home Health, Community Health services or Discharge Planning Preferred 
  • High School Diploma or equivalent Required
