Position Summary
As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence. You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, individualized care.
Key Responsibilities
- Perform patient evaluations and develop individualized treatment plans
- Provide therapy to improve strength, mobility, balance, and overall function
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Communicate with physicians, residents, families, and care team members
- Participate in care planning and discharge planning
- Conduct staff education and in-service training
Qualifications
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Active, unencumbered Physical Therapist license in South Carolina (required)
- Must maintain all continuing education and remain in good standing with the State Board
- Experience in a long-term care or rehab setting preferred
- Familiarity with PointClickCare (PCC) and Casamba preferred
- Strong clinical, communication, and organizational skills
