Reentry Specialist - Delaware County

PHMC’s Forensic Services department is looking for a full-time Reentry Specialist to provide support to Delaware County Reentry Project (DCRP) participants. The Reentry Specialist serves a vital role in bridging the gap between incarceration and successful community reintegration. The Reentry Specialist provides credible, empathetic, and strengths-based support to justice-involved individuals. Working collaboratively with probation officers, the DCRP Case Manager/Behavioral Health Assessor, and Peer Recovery Specialists, the Reentry Specialist assists with individualized transition planning that addresses housing, employment, behavioral health, recovery supports, and compliance with supervision requirements.
