Reporting to the Director of Admissions or Clinical Manager – Admissions, the RN Admission Liaison Nurse plays a critical role in ensuring a smooth and compassionate transition into hospice care. This position is responsible for conducting patient assessments and facilitating admissions while ensuring continuity of care with the interdisciplinary team (IDT).
You will:
- Conduct comprehensive patient assessments to determine eligibility and appropriateness for hospice care
- Coordinate and facilitate seamless admissions into hospice services
- Provide education to healthcare professionals, patients, and families on hospice philosophy, services, and benefits
- Build and maintain strong relationships with referral sources to support growth and access to care
- Actively identify and develop new referral opportunities while strengthening existing partnerships
- Collaborate closely with internal clinical teams to enhance service delivery and patient outcomes
- Serve as a key liaison between Care Dimensions and hospitals, facilities, and community providers
What You’ll Bring
- Current Registered Nurse (RN) license in Massachusetts in good standing
- Associate’s degree in Nursing required; Bachelor’s degree (BSN) preferred
- Minimum of 3 years of nursing experience, including at least one year in an acute care setting (preferred)
- Strong clinical assessment and decision-making skills
- Demonstrated ability to support patients and families through complex and emotional situations
- Understanding of hospice philosophy and end-of-life care principles
- Highly self-motivated, flexible, and collaborative team player
- Strong communication and relationship-building skills
- Valid driver’s license with reliable, insured transportation

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post.
