Full-Time position available for a Pathways Community Health Workerfor our Care Management team in Vancouver, WA. This position worksclosely with all clients, community organizations, and otherdepartments. This culturally and geographically connected individualserves as a link between underserved communities and existingcommunity resources. Candidates with a background in communityoutreach or case management are preferred. Knowledge of communityresources is preferred. Through home visiting, this individual assistsclients in overcoming barriers to health, social services, educationand employment. Candidates who are bilingual in Russian or Spanish arehighly preferred.Driving to meet clients in homes, clinics, and hospitals is required.This person must have a valid driver license, proof of auto insuranceand a vehicle safe for daily use. Prior to hire this person mustsubmit a driver's abstract demonstrating that they are safe to driveas deemed by Sea Mar's Health and Safety/Compliance Officer. Withinthe course of employment, a current driver's abstract may berequested. DUTIES AND RESPONSIBILITIES: * Be a patient advocate. * Conduct standardized patient in-take and risk assessments. * Establish a trusting relationship with the target population,referred clients, and community partners. * Identify and assist clients with client identified health andsocial service goals as appropriate. * Assist in the development of strategies to address or eliminatebarriers to care. * Work collaboratively with the Program Manager, PCP, and otherassigned personnel to make sure that all relevant client needs areidentified and addressed. * Participate in on going assessment of the client and family needsto determine if expected outcomes are reached and to ensureimplementation and/or adjustment of the service plan. * Providing ongoing follow-up on treatment on PCP plans utilizingbasic motivational interviewing and goal setting withclients/families.REQUIREMENTS AND/OR EXPERIENCE: * High School Diploma or Equivalent required. * Community outreach or case management experience preferred. * Experience working with people from various backgrounds required. * Experience working with individuals with substance use disorders,homelessness, chronic mental illness, or people in crisis preferred. * Maintain a clear understanding of the Federal, State, and communityresources available. * Demonstrate the ability to recognize potentially dangeroussituations to self in the clinic,office, home and community settings and to take appropriate action. * Prefer one year paid or volunteer experience providing medical,social service or behavioral health case management or carecoordination. * Must have current CPR certification and maintain throughoutemployment. * Bilingual English/Spanish or English/Russian is preferred, but notrequired.Hourly - Hourly Plan, 22.40 USD HourlyWhat We Offer:
