Care Coordinator II or III RN or Licensed SW (7 county metro area in MN)

Medica’s Care Coordinator position is critical to the Medica Care System and is responsible for assessing the medical and social needs of the Medica Care System products’ members and ensuring they receive all the necessary care and services they require. The assessments occur in the community, at facilities or via telephone.  Assess medical, behavioral, mental health and social needs of members to determine, create and manage individualized care plans and coordinate the delivery of medical and social services. Provide recommendations for Medica's care coordination program.
