
Guardianship Representative

Department: Guardianship
General Description:
Responsible for general oversight, case management and documentation of guardianship case load for persons deemed incapacitated and supported through the guardianship program. This will also include duties of Rogers monitoring.
Minimum Qualifications:
- Bachelor's degree in the human services/social sciences field or demonstrated knowledge and skill equivalent to the completion of four years of college.
- Two years' experience working with individuals with developmental and/or intellectual disabilities preferred.
- Knowledge of DDS service delivery system.
- Ability to utilize computer documentation program including producing invoices, plan of care reports and case note documentation.
- General knowledge of federal and state disability laws as well as a working knowledge of community systems and services in the area to be served.
- Ability to pass driving record and criminal background checks.
Job Responsibilities:
- Coordinate and acts on behalf of eligible disabled adults in personal, medical and estate matters as appointed by probate court.
- Assures that treatment provided is in the best interest of person, including medical, surgical, hospitalization and all other service providers required by the level and type of guardianship provided.
- Responds to telephone inquiries from hospital staff and long-term care staff concerning consent requests for emergency medical procedures and hospital admission for individuals; consults with hospital staff and other professional staff about unusual situations affecting individuals.
- Maintain open communication with all services providers involved with individual supported by guardianship program.
- Prepare and maintain case records, activity reports/statistics, court plan of care reports and other required reports and forms.
- Completes required Roger's monitoring duties as assigned.
- On call duties as needed per guardianship case load requirements.
- Performs other duties as required or assigned by Director of Guardianship.

The employee is regularly required to use hands in order to handle or feel objects, tools, and controls; reach with hands or arms, stoop, kneel, talk, and hear. The employee is required to stand, walk, sit, and occasionally be required to lift/transfer up to 50/70 lbs. alone or occasionally up to 200lbs with the assistance of other staff. Pay Range: $24 - $25 per hour
