
Patient Nurse Navigator
Job Description
When a patient requires care from different facilities and/or practitioners they need a Patient Nurse Navigator to identify needs and remove any barriers to timely, appropriate care. Our Nurse Navigators guide the patient from diagnosis through treatment and act as the central point of contact for the patient and the entire medical team. It’s a job that requires a strong communicator with excellent organization skills.

WHAT YOU CAN EXPECT
Monday to Friday, salary position 8am-4:30pm, with no weekends or holidays
Cardiac / Heart Failure Nurse Navigator
Provide clinical resource guidance on where to seek a second opinion, treatment or testing that may not be available locally, and how to access reliable sources of information.
Assess patients for psychosocial distress and referral to a medical social worker or counselor if support that is more intensive is required.
Address tangible barriers to care such as finances, housing, transportation, childcare, prescription costs, and much more. Refers patients to the appropriate contact, such as a social worker, for support needed.
Function as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services through the continuum of care.
Empower patients, families, and caregivers to assume as much responsibility for their care as possible, within the constraints of age, illness, and other factors.
Educate patients about their disease, prescribed treatment, and side effects of treatment and how to manage them.
Qualifications
Required Associate's Degree Nursing / Patient Care
Preferred Bachelor's Degree Nursing / Patient Care
1 year related experience
PreferredRegistered Nurse (RN) - State Licensing Board
Basic Life Support Program (BLS) - American Heart Association
Unit Specific Certifications - Franciscan Alliance
