Monitor resident health and wellness

The responsibility of the Wellness Nurse (WN) is to supervise delivery of care and services to residents. In achieving this goal, the WN performs procedures within the guidelines of all state and local regulations and accepted standards of care. Ongoing communication between the WN, associates, the residents and their families, professional consultants, and third-party providers is critical to the welfare of the residents.
Areas of Responsibility
Provide daily supervision and leadership to assigned non-licensed associates and other designated personnel in meeting the residents needs and monitor the condition of residents in AL. Demonstrate good decision-making skills.
Make work assignments and meet with associates regularly to assist in identifying and correcting problem areas, and/or the improvement of care and services.
Provide resident care according to health care providers orders and in accordance with regulatory requirements, recognized nursing practice, established standards, and Company policy.
Accurately administer prescribed medications, supervise the continuity of resident care, respond to changes in resident condition, medications, or other circumstances requiring special care needs and perform first aid as required.

Required Qualifications
Must be a minimum of 18 years of age.
Licensed Practical Nurse in good standing with the state licensing agency, as required by state regulations.
CPR/AED certified.
Must have at least one (1) year of experience working as an LPN in a senior care setting.
Must have a positive Criminal Background Screening.
The Community is a drug free workplace; associates are subject to adherence to the Company Drug Free Workplace policy.
Previous experience with computerized resident documentation systems a plus.
