Job Summary
Summary:Responsible for assisting patients to arrange transportation to/from the hospital for ongoing medical treatment and/or to locate affordable accommodations and support groups. Works closely with the unit-based case manager to facilitate discharge plans to other psychiatric or medical settings. Other responsibilities may include making referrals to and organizing and disseminating information about a range of other related social resources.

Essential Functions:
-Researches information on community social resources.
-Maintains files of brochures, information packets and applications for all types of community resources.
-Collaborates with social work staff and personnel from community agencies to obtain needed information.
-Assists in planning/implementing in-service presentations by and about community resources.
-Assesses patient/family understanding of available resources.
-Educates patients/families about appropriate resources and how to access them.

High School Diploma or Equivalent required or Bachelor's Degree preferred

The Case Management Resource Specialist supports RN Case Managers with hospital discharge planning. This is not a social work role, but a support position focused on coordinating patient transitions of care.
This role is both patient-facing and administrative — you’ll round on units, visit patient rooms to provide Medicare documentation, and assist with coordinating services like rehab, home care, and medical equipment.
Candidates must be comfortable navigating multiple systems, working in a fast-paced environment, and interacting with patients and care teams. A Bachelor’s degree is strongly preferred.
Schedule: Monday–Friday, with 1 weekend day/month and 2 holidays/year. Hours are typically between 8:30 AM – 5:00 PM.
