Position Summary

The Advanced Heart Failure Program is seeking a dedicated and experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to provide comprehensive care to patients with complex heart failure and those undergoing evaluation for advanced therapies, including heart transplantation. The APP will work collaboratively within a multidisciplinary team to deliver exceptional patient care in the outpatient setting.


Key Responsibilities

  • Provide comprehensive assessment and management of patients with advanced heart failure, including medication optimization and adjustment of guideline-directed medical therapy (GDMT), diuretics, and other therapies.
  • Order, interpret, and manage diagnostic studies, including laboratory tests, echocardiograms, and other cardiac imaging.
  • Educate patients and their families regarding disease progression, treatment options, lifestyle modifications, and individualized care plans.
  • Participate in heart transplant evaluations by gathering clinical data, coordinating diagnostic testing, documenting findings, and presenting cases during multidisciplinary transplant selection meetings.
  • Support the optimization and management of patients being considered for heart transplantation and other advanced therapies.
  • Collaborate closely with physicians, nurses, transplant coordinators, social workers, and other members of the care team to ensure safe, efficient, and patient-centered care.
  • Contribute to quality improvement initiatives, clinical program development, and process enhancement efforts.

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Active state licensure and national certification as a Nurse Practitioner or Physician Assistant.
  • Current BLS and ACLS certifications required.
  • DEA registration, as applicable.
  • Preferred experience in cardiology, advanced heart failure, inpatient cardiac care, or transplant evaluation.
  • Strong clinical judgment, communication skills, and the ability to thrive in a fast-paced, multidisciplinary environment.

Schedule

This position is primarily outpatient-based and involves managing patients in the ambulatory setting while working closely with the Advanced Heart Failure team.
