The Medical Case Manager serves as a key member of the Early Intervention Program (EIP) care team, supporting individuals living with HIV and other medically and socially complex patient populations through comprehensive care coordination, linkage to services, and patient engagement activities.The Medical Case Manager collaborates closely with physicians, nurses, behavioral health providers, and community-based organizations to assess patient needs; coordinate medical, behavioral health, and supportive services; facilitate appointment scheduling and follow-through; and assist patients in addressing barriers to care including housing instability, transportation, insurance, behavioral health needs, and other psychosocial challenges.
The Medical Case Manager is responsible for timely and accurate documentation in accordance with Ryan White, HRSA, Jefferson, and departmental requirements and supports delivery of the Jefferson Experience through professional, compassionate, and responsive patient-centered care.
Job Description
Clinical/Care Coordination:
- Coordinate medical case management services for patients enrolled in the Ryan White Early Intervention Program (EIP)
- Conduct patient outreach, engagement, intake, reassessment, and follow-up activities
- Assist patients with linkage and retention in HIV medical care and supportive services
- Coordinate referrals for behavioral health, housing, transportation, substance use treatment, benefits enrollment, dental care, specialty care, and other community resources
- Collaborate with multidisciplinary clinical teams during huddles and clinic sessions to support patient care needs and workflow
Documentation/Compliance:
- Maintain timely, accurate, and compliant documentation within Epic, CAREWare, and other required systems
- Ensure compliance with Ryan White, HRSA, HIPAA, and organizational documentation standards
- Assist with audit preparation activities and operational reporting requirements as needed
Workflow/Responsiveness:
- Monitor provider referrals, patient needs, and assigned caseload to ensure timely follow-through and resolution
- Respond promptly to provider, patient, and operational requests within a fast-paced ambulatory environment
- Participate in clinic workflow activities, including occasional evening clinic support as operationally required
Field/Community Work:
- Participate in field-based outreach, home/community visits, and patient engagement activities as needed
- Maintain professional boundaries and adherence to organizational policies during field activities and patient interactions
Minimum Qualifications:
- Bachelor’s degree in social work, psychology, public health, health administration, or related field preferred
- Minimum 3 years of experience in medical case management, care coordination, ambulatory healthcare, Ryan White services, HIV care, behavioral health, or related healthcare setting preferred
- Experience working with medically and socially complex patient populations strongly preferred
- Strong organizational, documentation, and follow-through skills required
- Ability to work collaboratively in a fast-paced clinical environment and respond to changing patient and operational needs
- Strong verbal and written communication skills required
- Ability to maintain professionalism and effective working relationships with patients, clinicians, staff, and community partners
- Reliable transportation required for community-based activities
Preferred Experience:
- CAREWare experience preferred
- Ryan White / HIV experience preferred
- Epic experience preferred
- Bilingual skills preferred
Important Additions:
Ability to work effectively with patients experiencing behavioral health conditions, housing instability, substance use disorders, trauma, and other complex psychosocial barriers.
Demonstrated ability to manage competing priorities while maintaining responsiveness, professionalism, and timely follow-through in a clinical environment and during field work.
Salary Range
The actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
