Social Worker-PACE (Murrieta)

Join Neighborhood Healthcare as a Social Worker and make a meaningful impact through compassionate care. In this vital role within the PACE program, you’ll manage a caseload of participants, bringing a psychosocial lens to assessments, care planning, and ongoing support. Collaborating closely with an interdisciplinary team, you’ll help enhance the health and quality of life for older adults, while serving as a key liaison between participants, their support networks, and care providers.
