Position: Community Health Worker (Must have current CHW certification to be considered for this position!)
Company/Location: Kentucky River Community Care/Perry County Outpatient, Hazard, KY
Employment type: Regular Full-Time
Credentials: High School/GED preferred and Community Health Worker certification REQUIRED.
Special skills: Medical knowledge and ability to work with an Electronic Medical Record.
Primary functions:
- Provide health education and information with clients.
- Coordinate care between individuals, communities, and health and social service systems.
- Be a liaison between providers and clients.
Shift/Days and Work Hours: Monday – Friday, 8:00am – 4:30pm
