Patient Care Coordinator LPN

- Manage centralized medication processes - Completing medication prior authorizations and peer reviews or appeals for medication denials to facilitate provider written treatment plan and improve patient care - Completing centralized medication refills following written protocols that include evaluation of patients chart for appropriate testing and follow up - Manage transitions in care for patients and their care givers - Manage chronic care management as directed by the patient, their care giver or physician/APP - Perform periodic outreach to patients/caregivers and communicate with physician/APPs and RN Care Coordinator on behalf of the patient/ patient care giver - Participate in maintaining and developing quality and performance initiatives...
