Transition Facilitator

Division/Program:

In Home Outreach Team - Alameda

Starting Compensation:

22.36 - 26.83 USD Per Hour

Working Location:

San Leandro: San Leandro Center (In-person position)

Working Hours/Shift:

FT, Monday-Friday, 8:00 am-5:30 pm

What you bring to SBHG (qualifications):

Education

  • High School Diploma or equivalent 
  • Must be enrolled in an Associate’s program 

Licenses and Certifications

  • Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.

How you will make a difference (job overview):

This position has primary responsibility for mental health services and case management services to clients and their families within Stars Community Services.   

Division/Program Overview:

  • HOT team provides outreach and engagement services to disengaged TAY (transitional ages youth)  
  • Transitional Age Youth (TAY) from Ages 18-25 years. 
  • The IHOT team is comprised of a staff of peer advocates who use their lived experience to help them connect with disengaged youth. 
  • The team lead provides clinical oversight to this dedicated staff. 
  • Provided community-based services throughout Alameda County.
