On-Call Children's Case Manager Assistant (Support Provider II)

Job duties includes: - Works one-on-one with the youth and family to provide the services needed to resolve issues and stabilize behavior - some examples would include teaching the youth skills necessary for successful relationships, implementing strategies with the youth and family for school success and working on developing independent living and self-sufficiency skills. - Facilitates Psychosocial Groups on a variety of topics including: problem solving, social skills, leisure time training, health and personal relationships. - Provides crisis interventions as necessary, which may including physical intervention and restraint to ensure the safety of the child. - Transports individuals receiving services to appointments and...
