McClure Post Acute is seeking a compassionate and motivated Speech Therapist (ST) to join our rehabilitation team on a PRN (Per Diem) basis. This is an excellent opportunity for a therapist who is passionate about helping patients improve their communication, cognition, and swallowing abilities while working alongside an experienced interdisciplinary team dedicated to exceptional patient outcomes.
Position Summary
As a Speech Therapist, you will evaluate, diagnose, and treat residents with speech, language, cognitive, voice, and swallowing disorders. You'll play a key role in developing individualized treatment plans that promote recovery, maximize independence, and enhance quality of life.
Responsibilities
Evaluate residents' speech, language, cognitive, and swallowing function
Develop and implement individualized treatment plans
Provide evidence-based therapy to improve communication and swallowing abilities
Collaborate with physicians, nursing staff, therapy team members, and families to coordinate care
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely
Educate residents, caregivers, and staff on therapy goals and home exercise programs
Maintain compliance with Medicare, state, and facility documentation standards
Participate in interdisciplinary care planning meetings as needed
Qualifications
Current Speech-Language Pathologist license in the State of California
Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellows may be considered
Current CPR/BLS certification (or willingness to obtain)
Experience in skilled nursing or post-acute care preferred
Strong communication, organizational, and clinical skills
Ability to work independently while collaborating effectively with the rehabilitation team
