
RN Congregational and Community Nurse - Part Time
Job Description
Essential Job Function
- Assists community members in finding a primary care provider in order to connect them with access to care.
- Supports the health and wellness of members through personal health counseling to develop care plans.
- Identifies health disparities, including transportation, housing, and food to remedy issues that are preventing members from improving personal wellness.
- Educates members on disease prevention and management, medications, and the health system in order to increase knowledge and awareness.
- Organize health screenings and fairs and flu clinics to support the reduction of illness in the community.
- Completes screenings and assessments such as blood pressure and blood glucose monitoring to identify potential health problems for treatment.
- Provides spiritual care through prayer and listening in order to support a holistic lifestyle.
- Performs other duties as assigned.
Education
- Required: Associates Degree in Nursing
- Preferred: Bachelors Degree in Nursing
Experience
- Required: 5 years of clinical experience in a health care setting
Preferred:
- Bilingual: Spanish
- Occasional weekends and evenings
Licensure/Certification/Listing
- Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider
