Position Summary
We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy services on a PRN basis. This role focuses on helping residents improve mobility, strength, and independence while working as part of an interdisciplinary care team.
Key Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled therapy services per physician orders and care plans
- Document evaluations, treatments, and progress notes in compliance with regulations
- Communicate effectively with physicians, nursing staff, and care team members
- Supervise PTAs, aides, and students in accordance with state practice acts
- Participate in care planning and interdisciplinary meetings
- Educate residents, families, and caregivers on therapy goals and discharge plans
- Conduct staff training and in-service education
- Maintain compliance with all state, federal, and payer guidelines
Qualifications
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Active South Carolina Physical Therapist license (in good standing)
- Experience in skilled nursing or rehabilitation setting preferred
- Familiarity with PointClickCare and Casamba preferred
- Strong clinical, documentation, and communication skills
