How you’ll make an impact in this role
This is an exceptional opportunity for a motivated Nurse Practitioner to join an employed position within a busy, well-respected geriatric care facility, supported by a strong interdisciplinary team and the full resources of a robust health system network.
- Join an established team of 1 physicians and 6 APPs
- Call structure: Light 1:7 weekend call, very rare weekday calls, providing guidance to staff (not required to come in)
- Practice setting: Longitudinal Primary Care/PACE Model, managing a smaller patient panel
- Access to Strong interdisciplinary support team
- Employed position within a nonprofit health system
- Board Certified or Board Eligible as a Certified Nurse Practitioner (NP)
- Eligible for state APRN licensure in Kansas and DEA registration
- Provide comprehensive, value-based primary care and case management to frail elderly participants in the PACE program, focusing on quality outcomes
- Collaborate closely with the interdisciplinary team (IDT) to ensure patient-centered decisions and holistic care

What minimum requirements you’ll need
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Nurse Practitioner obtained prior to hire date or job transfer date required.
- Registered Nurse obtained prior to hire date or job transfer date required.
Education:
- Master's degree of Nursing required.
