Case Management Physician

- Round on assigned inpatient service with a typical census of 15–17 patients - Manage up to a maximum census of 22 patients - Perform new admissions from 7:00 AM – 4:00 PM - Average 12 patient encounters per day - Collaborate closely with hospitalists, nursing, and care coordination teams - Support medical necessity reviews and appropriate level-of-care determinations - Assist with discharge planning and reducing length of stay
