The Care Team Hospice is seeking a compassionate and dependable Part-Time Certified Nursing Assistant (CNA) to join our team! This is a rewarding opportunity for a caregiver who is passionate about providing hands-on support and making a meaningful difference in the lives of patients and families.
As a Hospice CNA, you'll work closely with patients in their homes and care facilities, assisting with personal care needs while providing comfort, dignity, and companionship. You'll be an important part of the interdisciplinary care team, helping patients maintain the highest possible quality of life throughout their hospice journey.

Schedule & Territory:
• Primary schedule: Part-Time/As Needed/Flexible to your schedule
• Territory: Clare/Mt. Pleasant
Key Responsibilities:
• Provide personal care services including bathing, grooming, dressing, toileting, transfers, and mobility assistance.
• Support patients with activities of daily living while promoting comfort, dignity, and independence.
• Observe and report changes in patient condition to the clinical team in a timely manner.
• Provide compassionate emotional support to patients and families throughout the hospice journey.
• Maintain accurate and timely documentation while adhering to hospice policies, HIPAA requirements, and safety standards.
Qualifications:
• Active CNA certification required.
• High School Diploma or GED required.
• Graduate of an approved nursing assistant training program.
• Hospice, home health, long-term care, or direct patient care experience preferred.
• Valid driver's license, reliable transportation, and current automobile insurance required.
