WRAPAROUND FACILITATOR

 
 
    
Position Summary 
 
Performs tasks including linkage, referral, crisis management, and follow-up.  A facilitator provides services that are goal-oriented activities that assist clients by locating, coordinating, and monitoring necessary care and services insuring that they are appropriate and accessible. The facilitator coordinates building family teams for each client.  Work with clients occurs in a variety of settings.

  • Consistently exercises discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Works without daily and immediate supervision, evaluating possible courses of conduct and making decisions where there is no opportunity to seek supervisory assistance.
  • Consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining goals.
  • Maintain expected productivity and an active caseload providing facilitation as needed and within area of expertise and limits of credentials.
  • Provide direct services to referred children/families.
  • Assess the strengths and needs of project families, facilitate the development of an individualized family service plan and monitor the progress in meeting established goals.
  • Assist families with accessing community resources.
  • Provide individual case management and activity of daily living services as needed.
  • Provide crisis management for clients; make linkages for interventions as appropriate. 
  • Maintain client and program records in accordance with applicable standards and regulations, grant requirements, etc.
  • Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
  • Participate in continuing education activities, remaining knowledgeable in area(s) of expertise.  Attend meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.
  • Provide grant funded services and comply with grant policy and procedures (i.e. PRTF Wavier and MFP grant, CMHW, CMHI, Prevention).
  • Completion of required CANS and/or ANSA training and ongoing certification including SuperUser certification.                                                                                                                   
  • Completion of required DMHA certification.
  • Other duties as assigned.
 
Minimum Qualifications/Requirements
 
  • Bachelor’s degree in Psychology, Social Work, Sociology, Family/Consumer Sciences, Child Development, Substance Abuse, Education, Criminology, Mental Health, Human Services, Counseling, or Therapy Fields required.
  • Two year of professional experience acquired post-degree in a private or public social services agency.
  • Two years of experience working with SED youth. 
  • Knowledge of child and adolescent growth and development and an ability to work with high risk youth and their families.
  • Good interpersonal and communication skills.
  • Willingness to use personal transportation in work capacity including transporting clients.
  • Familiarity with school and community resources and the ability to access them.
