Position Overview:

Provide high quality case management services, including but not limited to intake, case planning, court advocacy, cognitive behavioral therapy referrals and linkage to stabilization services. Additionally, leading case conferencing sessions with program coordinators and career coaches who also support the youth on your caseload. As well as facilitating additional projects that improve the performance of staff and/or benefit clients.

CHALK is a youth-led project that provides a range of youth services with a specific focus on

transformative youth development and employment.

Within CHALK there are 2 youth-led programs:

-YouthLine Tech: a job training program for youth interested in careers in technology and media.

-Career Pathways for Undocumented Youth: a career exploration program providing

undocumented youth with cohort based job training and career exploration opportunities to

various career pathways.

Senior Case Manager will be responsible for case managing all youth who are participants of CHALK programs.

Qualifications:

● Possession of bachelor degree or working towards degree in Criminal Justice, Psychology,

Sociology, or related field. Can be substituted with 4+ years experience working in youth

programs.

● Must have excellent oral and written communication skills

● Experience with workforce development programing, as well as education pathways

● Experience with immigration laws and policies and its impact on marginalized communities

● Experience working with undocumented and/or system impacted youth

● Must be familiar with restorative justice, trauma informed care, and motivational interviewing

● Ability to maintain confidentiality and demonstrate a high degree of integrity

● Must have valid California Driver’s License and insured, registered and operational vehicle to use

for work purposes

● Must pass a criminal background check and TB test clearance

● Availability to work in the evenings to meet with participants. Periodic Saturdays

● Ability to be on call

● Bilingual: Spanish preferred

Duties and Responsibilities:

● Conduct various assessments of youth/ TAY to identify and address wraparound barriers,

behaviors and corresponding characteristics that hinder stability and functionality

● Generate thorough individual service plans consisting of supportive services, resources,

referrals and linkage to promote stability and functionality

● Familiarity with identification documents from foreign countries and knowledge of how to

acquire them

● Request monthly cash advances to purchase supportive services and complete monthly expense

reports

● Build and maintain strong rapport with youth/TAY so participant identifies you as a key

figure in their supportive network

● Develop relationships and maintain communication with supportive network, providers and

stakeholders in efforts to case conference, strategize and better serve youth/TAY

● Collaborate with the Program Manager and Career Coach to facilitate trainings and workshops

and assist with planning career panels, information sessions, and field trips for CPU participants.

● Meet with youth/TAY on a weekly to monthly basis depending on the need to formally

monitor progress or regression of individual service plan and implement motivational techniques

to keep youth/TAY engaged

● Work with youth in setting and reaching self identified educational, professional and personal

long and short term goals

● Responsible for maintaining case files with detailed case notes and regular input of participant

information into database systems and spreadsheets

● Attend school meetings, court hearings, other necessary appointments and act as advocate

when appropriate

● Responsible for writing character, recommendation and progress letters/reports for court

and/or other institutions

● Represent CHALK at networking and partnership meetings and events

● Attend semi-monthly staff meetings for case conferencing and/or programmatic discussions

● Commitment to professional development by attending trainings and workshops to better serve

participants

● Other duties as assigned

Above is the pay range for this position that we reasonably expect to pay. Individual compensation is based on various factors including experience/education, skillset, and geographic location.
