Lead Patient Navigator

The Lead Patient Navigator is a member of the clinic interdisciplinary team responsible for leading the Patient Navigator team to improve the outcomes of the team. Responsibilities include: - Serve as a knowledge base and act as a resource for Patient Navigators - Train all new Patient Navigators, obtain access to all portals needed to complete Patient Navigator job duties - Provide up to date information to all Patient Navigators.
