Position Summary
We are seeking a licensed Physical Therapist (PT) to provide high-quality, patient-centered therapy services. This role is responsible for evaluation, treatment planning, team collaboration, and supervision of therapy staff to support residents’ recovery and mobility goals.
Key Responsibilities
- Evaluate residents within required timeframes and develop individualized treatment plans
- Provide therapy services in accordance with physician orders and care goals
- Document evaluations, treatments, and progress notes per regulatory standards
- Supervise PTAs, aides, and students in compliance with state practice act
- Collaborate with interdisciplinary team members on resident care plans
- Participate in care conferences, utilization review, and rehab meetings
- Educate residents, families, and caregivers on treatment plans and discharge needs
- Assist with durable medical equipment (DME) recommendations
- Provide staff training and in-service education
- Maintain compliance with all state, federal, and payer regulations
Qualifications
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Active South Carolina PT license in good standing
- Experience in skilled nursing or rehab setting preferred
- Familiarity with PointClickCare and Casamba preferred
- Strong clinical, leadership, and documentation skills
