RN Navigator - Cardiology

The Cardiac Nurse Navigator utilizes a systematic approach to nursing by incorporating assessment, planning, implementation, and evaluation in the coordination of care for patients with heart failure and cardiomyopathy. Working primarily in an ambulatory setting, the Cardiac Nurse Navigator provides ongoing monitoring of patient status, facilitates timely follow‑up, and supports patients and families across the continuum of outpatient cardiac care.This role collaborates closely with an interdisciplinary team, interfacing with providers to ensure seamless communication, appropriate escalation of care, and adherence to evidence‑based treatment plans.
