Job Description

Adventist Health White Memorial Montebello is looking for Sr. HIM Technician for Full-time, Day Shift and must be willing to work for Sunday to Thursday 8:30 to 5 PM. We are looking for great individual who can work onsite to our location in Montebello, CA

Job Summary:

Coordinates records availability, selection of incomplete records for delivery to surgery, and other units as needed. Uses the system to analyze records; filing documents in workroom records; and for filing incomplete records in the workroom. Applies substantial knowledge of the job and experience to complete a wide range of activities with varying difficulty. Regularly works with sensitive and confidential information, often involving the interpretation of policies and procedures to guide use.

Job Requirements:

Education and Work Experience:

  • High School Education/GED or equivalent: Required
  • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred
  • Two years' healthcare experience: Preferred
Essential Functions
  • Reviews incomplete medical record lists for discharge dates and researches discrepancies to ensure all discharged patient records are accounted for and processed timely.
  • Conducts daily rounds to patient care units throughout the hospital, walking to nursing stations and clinical departments to collect discharged patient documentation, records, and other required medical record components.
  • Carefully reconciles discharge lists against records located on patient care units, ensuring all required documentation is accurately identified, retrieved, and transported to the Health Information Management (HIM) Department.
  • Prepares, organizes, indexes, and scans medical record documents into the Electronic Medical Record (EMR) while maintaining the highest standards of accuracy, completeness, and confidentiality.
  • Performs quality control reviews of scanned records to verify image quality, correct patient identification, document indexing, and record integrity.
  • Meets established productivity, quality, and turnaround time metrics for record retrieval, preparation, scanning, and document management processes.
  • Demonstrates exceptional attention to detail when reviewing, auditing, and processing medical records to ensure compliance with organizational policies, regulatory requirements, and HIM best practices.
  • Audits the work of team members and identifies opportunities for corrections, coaching, process standardization, and quality improvement.
  • Proactively identifies workflow inefficiencies and recommends process improvements to enhance productivity, accuracy, customer service, and departmental performance.
  • Analyzes physician records for deficiencies and notifies providers of missing documentation in accordance with medical staff bylaws and regulatory requirements.
  • Updates tracking systems and facility databases when records are completed and resolves issues related to chart completion.
  • Processes, distributes, and follows up on physician deficiency notifications and correspondence while coordinating chart completion activities across multiple departments.
  • Maintains accurate physician suspension records and documentation, ensuring compliance with established suspension procedures.
  • Answers telephones, greets internal and external customers, and provides professional customer service assistance.
  • Assists physicians and hospital staff in locating records and obtaining access to medical record information as authorized.
  • Retrieves, files, and maintains incomplete, updated, and completed records in designated filing systems and storage locations.
  • Assists physicians with record completion needs and maintains an organized, clean, and professional physician work area with appropriate supplies and resources.
  • Maintains confidentiality and security of protected health information (PHI) in accordance with HIPAA and organizational policies.
  • Performs other related duties and responsibilities as assigned.
Preferred Knowledge, Skills, and Abilities
  • Strong knowledge of medical records management, document imaging, and Electronic Medical Record (EMR) systems.
  • Ability to work independently while managing multiple priorities in a fast-paced hospital environment.
  • Demonstrated accuracy and attention to detail when handling patient records and performing quality audits.
  • Ability to audit work for completeness, identify errors, and implement corrective actions.
  • Strong organizational, analytical, and problem-solving skills.
  • Ability to meet or exceed productivity, quality, and turnaround-time expectations.
  • Commitment to continuous process improvement and operational excellence.
  • Excellent communication and customer service skills.
  • Ability to perform frequent walking throughout the hospital, transporting records and documents between patient care units and the HIM Department.
