Summary:
This position provides comprehensive case management, discharge planning, admissions support, counseling, and utilization review for patients on the Substance Recovery Unit. This role ensures continuity of care through coordination with treatment teams, community providers, and payers to improve outcomes, appropriate length of stay, and timely transitions to community services
Qualifications:
| Education | Master’s preferred or Bachelor’s degree in social work, psychology ,counseling or a related human services field required
|
| Required Experience/Skills | Experience with payer authorization processes, utilization review, and a minimum of 2 years’ experience in substance use disorder treatment, counseling and case management services
|
| Preferred Experience/Skills | Strong knowledge of addiction treatment modalities, community resources, MAT, and behavior health
|
