Advanced Practice Provider (PA/NP) Medical Bridge Program

The Bridge Clinic Advanced Practice Provider will play a Critical role in this innovative care team focused on addressing health disparities in our community. Working closely with the Bridge Clinic Physician Lead and community partners, this provider will support initiatives to improve access and provide comprehensive care to marginalized individuals who have complex health needs with a special focus on individuals experiencing homelessness. Care will be provided both in a clinic setting collocated with multiple specialists, as well as a mobile setting alongside our community partners.
