• Coordinate patient intake an ensure smooth transition to home health

• Gather and verify patient information, history, and documentation

• Communicate with referral sources, patients, and internal teams

• Build and maintain strong professional relationships

• Support a fast-paced office with multiple priorities

 What You'll Bring:

• High School Diploma, some college preferred

• Medical certification or license is not required, but must have a background in the medical field

• 1+ years in Home Health preferred, medical terminology and basic knowledge of different diagnoses required for consideration

• Excellent communication, organizational skills

• Ability to multitask and manage schedules, projects, and reports

• Ability to build relationships, problem solve, and thrive in a fast-paced environment.

• Proficiency with MS Office products such as: Word, Excel, PowerPoint, Outlook, etc. and EMR systems
