What You'll Do:
Coordinate patient intake an ensure smooth transition to home health
Gather and verify patient information, history, and documentation
Communicate with referral sources, patients, and internal teams
Build and maintain strong professional relationships
Support a fast-paced office with multiple priorities

What You'll Bring:
High School Diploma, some college preferred
Medical certification or license is not required, but must have a background in the medical field
1+ years in Home Health preferred, medical terminology and basic knowledge of different diagnoses required for consideration
Excellent communication, organizational skills
Ability to multitask and manage schedules, projects, and reports
Ability to build relationships, problem solve, and thrive in a fast-paced environment.
Proficiency with MS Office products such as: Word, Excel, PowerPoint, Outlook, etc. and EMR systems
