Job Summary
We are seeking a dynamic and results-driven Hospital Liaison to lead and coordinate the referral process within an assigned market. In this role, you will evaluate patient needs—both clinical and financial—and collaborate with referral sources to ensure appropriate placement in our long-term care facilities.
The ideal candidate is relationship-focused, highly organized, and experienced in healthcare marketing, case management, or admissions.
Key Responsibilities
Coordinate and manage the referral process to support facility census and quality mix goals
Build and maintain strong relationships with hospitals, physicians, case managers, and referral sources
Respond promptly to referral inquiries and complete patient assessments (goal: within 30 minutes)
Collaborate with Admissions Directors on benefits verification and pre-certification
Provide recommendations for admissions based on clinical and financial evaluations
Communicate admission decisions and coordinate smooth patient transitions into facilities
Maintain accurate records in CRM/database systems to track referrals and activities
Work closely with facility leadership (Administrators, DONs, Business Office Managers)
Monitor bed availability, waiting lists, and patient placement needs
Stay informed on Medicare, Medicaid, Managed Care, and regulatory requirements
Support marketing initiatives and assist with stakeholder onboarding
Participate in process improvement efforts to enhance workflow and customer experience
Qualifications
RN license preferred; LPN or other healthcare backgrounds considered
Minimum 3+ years experience in long-term care or related healthcare field
1–2 years of sales, marketing, or case management experience preferred
Strong understanding of referral processes and long-term care operations
Excellent communication and relationship-building skills
Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook)
Strong organizational skills with the ability to manage multiple priorities
Ability to work independently and make informed decisions in a fast-paced environment
Work Environment
Work conducted in hospitals, healthcare facilities, and community settings
Interaction with patients, healthcare staff, and families
Exposure to clinical environments, including potential health risks and high-stress situations
Physical Requirements
Ability to stand/walk for extended periods (4+ hours/day)
Ability to sit and work on a computer for extended periods
May require lifting up to 25 lbs
