FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country. We operate more than 90 sites across San Diego County, including 29 primary care clinics, 23 behavioral health facilities, 10 physical rehabilitation clinics, nine dental clinics, five vision clinics, four outpatient substance use treatment programs, three mobile medical units, two mobile counseling centers, two urgent care centers, and a pharmacy.
Our staff provides care to over 227,000 patients each year, of whom 91% are low-income and 29% are uninsured. FHCSD provides care to all. Services include, but are not limited to adult care, chronic disease management, pediatrics, comprehensive women’s care including obstetrics, dental, vision, case management, physical rehabilitation, speech therapy for children, vaccinations, infectious diseases, behavioral health, substance use counseling and a host of specialty services including cardiology, podiatry, endocrinology, dermatology, among others. FHCSD also offers supportive services to those who are unsheltered and in need of intensive case management.

General Job Description
Under the supervision of the HIM Coordinator the Health Information Management Technician II will perform a variety of correspondences, document management, archiving, Quality Assurance/Quality Control and data abstraction activities.
Job Roles
- Answers phone calls, greets visitors, takes messages, and routes faxes.
- Assists in paper medical record inventory, purging, and shredding activities at both archive and off-site storage areas.
- Coordinates the release of medical information following department and regulatory guidelines. Verifies authorization of incoming record requests, completes processing, and communicates with requestor.
- Ensures applicable regulatory and department standards are met or exceeded and adhered to on an on-going basis (JCAHO, OSHA, Safety Standards, etc.)
- Ensures records requiring Provider review and approval are scanned, uploaded, and messaged to providers within 48 hours.
- Files and retrieves medical records. Attaches loose reports to correct charts, verifying data. Maintain logs and billing systems for PHI charges.
- Performs other duties as assigned.
- Reviews medical records for accuracy and completeness. Participates in a variety of Quality Assurance, Quality Control, and data abstraction as assigned.
Education/Certifications/Licenses/Registrations
- Associate Degree preferred.
- At least 2 years experience in a Medical Record or HIM environment required.
- Experience with record/chart analysis and immunization extraction is preferred.
- High school diploma (or GED equivalent) required.
- HIT or RHIT certification preferred.
- Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
Experience/Specialized skills (including Language)
- Basic computer literacy (i.e., use system to manage and schedule appointments, access electronic medical record information).
- Bilingual in Spanish/English preferred.
- Intermediate interpersonal and customer service skills.
- Intermediate written and verbal communication skills.
- Knowledge of medical terminology and EHR structure.
