- Performs other duties as assigned.
- Navigates neurosurgical inpatients from admission through treatment and discharge, ensuring timely medical and supportive services.
- Initiates patient coordination at the entry point into the system and prepares patient cases for pre-procedural review with a multidisciplinary team.
- Formulates individualized care plans based on therapeutic principles appropriate for age and psychological development, evaluates the effectiveness of interventions, and modifies plans as needed.
- Ensures patients' physical, mental, and spiritual needs are met by coordinating with appropriate care team members and maintains a system for tracking the continuum of care.
- Assumes responsibility for professional growth by attending mandatory in-services, maintaining knowledge of current trends, practice changes, and research findings, and networking with other professionals.
- Assists with program development, creating a system-wide program that expands the referral base, and is actively involved in marketing strategies and meetings.
- Establishes relationships to maximize program effectiveness and facilitates problem resolution across departmental, campus, and physician/provider service lines.
- Communicates effectively with physicians, healthcare teams, patients, and families to ensure coordinated and cohesive execution of quality patient care.
- Serves as a patient resource and advocate, promoting access to available services, tools, and support, and develops, coordinates, and documents patient education.
- Performs follow-up phone calls after discharge, contributes to post-acute care planning, assesses patients' understanding of discharge plans, and provides education as needed.
- Gathers, abstracts, and reviews data for process and quality improvement, analyzes practices contributing to variance, and assists in developing action plans using best practices and research findings.
- Researches and responds to Neuro-Oncology Tumor Board information requests, records conversations at weekly conferences, and follows up with outcomes as necessary.
Knowledge, Skills, and Abilities:
• Advanced knowledge of adult neuroscience, neuro-interventional, neurovascular, brain, and stroke related treatment options [Required]
• Skilled in providing age-appropriate education and patient care by interacting positively with patients, families, staff, physicians, and the community [Required]
• Strong communication and collaboration skills [Required]
• Maintains the confidentiality of patients, physicians, and department information with no infractions [Required]
• Aptitude for critical thinking, decision making, and managing competing priorities [Required]
• Ability to interact appropriately with all other departments regarding care management and illness adjustment issues [Required]
• Skilled in self-direction and organization. Effective decision-making skills with minimal supervision [Required]
• BLS, ACLS [Required]

Education:
• Bachelor's of Nursing [Required]
Work Experience:
• 3+ experience in neuroscience nursing or related healthcare field [Required]
Licenses and Certifications:
• Registered Nurse (RN) [Required]
• Certified Neuroscience RN (CNRN) [Preferred]
• Acute/Critical Care Clinical Nurse Specialist (CCNS) [Preferred]
• Basic Life Support - CPR cert (BLS) [Required]

Pay Range:
