Community Health Network
CHN is seeking a Full or Part Time RN case manager for Wayne County.
Please apply to discuss wage, Indeed projected wage inaccurate at a lower rate.
Case Managers are responsible for completing OASIS assessments (Start of Care, Recertification, Discharge, Resumption of Care) as well as Skilled Visits, Med Passes, and Supervisory Visits. On-Call rotation required.
