
Case Management Coordinator - Assertive Community Treatment (ACT)
Job Description
Assesses and evaluates the needs of ACT individuals and continues caseload contact to develop goals. Supports individuals in developing appropriate program plans to meet identified needs. Completes referral forms, clinical and legal documentation necessary to obtain community based services for individuals. Advocates for new services as needed. Seeks out and develops community resources to meet the needs of assigned individuals for activities and support services.
