Field Liaison

The Field Liaison will be responsible for coordinating services and identifying patients through the health system that may qualify for post hospitalization services. The Field Liaison will assist with identifying patients and assist the hospital with referrals and prior authorization for services aligned with facility standards. The Field Liaison develops relationships and serves as an educational resource for referring providers, patients and family members. The Field Liaison coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care.
