THIS IS A COLLECTIVE BARGAINING UNIT POSITION

Provide care coordination through the Healthy Start Program for program-eligible pregnant women to help ensure these women have a medical home for prenatal and post-partum care, practice appropriate pregnancy self-care, get connected to needed social services to remove barriers to care, and learn basic maternal and baby care with the goal of achieving a full term, healthy birth weight birth.

EDUCATION

  • Requires a high school diploma or equivalent and knowledge of community resources.

  • Prefer a Certified Community Health Worker.

QUALIFICATIONS:

  • Must have a valid Ohio driver’s license and automobile insurance.

  • Must be able to share educational information and resources with individuals and families.

  • Must be culturally sensitive to families and demonstrate respect in all interactions.

  • Must maintain confidentiality at all times.

  • Must have good communication skills.

  • Must be a self-starter
