Neighborhood Healthcare is seeking a Complex Care Resource Specialist to help coordinate patient referrals and ensure seamless follow-up care for individuals with chronic conditions and social needs. In this role, you’ll be the vital link connecting patients to essential resources, supporting their health journey every step of the way. You’ll collaborate across teams, travel to NHC sites for meetings and staff training, and play a key part in advancing our mission to provide compassionate, quality care to everyone.

Work schedule: M-F/8am-5pm.
