Hourly Compensation Range $29.00-$36.00

40 hours +Facility shifts times for this Position :

9am-5pm Week 1: Monday, Tuesday, Thursday, Friday, Saturday

9am-5pm Week 2: Monday, Tuesday, Wednesday, Thursday, Friday

Successful candidates will have the following:

Current, unencumbered license to practice as a nurse in PA and will remain in good standing with the State Board of Nursing
Ability to maintain all required continuing education
Must be 18 years of age or older
Clear Cisive Background Check- last 7 years reviewed
Active CPR Certification (BLS PREFERRED with Skills)
Must meet the general health requirements set forth by the policies of this facility, which include a physical examination and 2-step PPD or equivalent. 
While performing the duties of this job, the employee is regularly required to sit, stand; walk and talk, read or hearMust be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. 
Experience with Point Click Care is preferred
Compassion for a vulnerable population and willingness to advocate for patients
Understanding of Patient Rights
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.

Must not pose a direct threat to the health or safety of other individuals in the workplace. Must be able to cope with the mental and emotional stress of the position.

General Purpose

The primary purpose of your job position is to provide and coordinate treatment services to residents, including wound care, skin integrity management, and respiratory treatments and assessments, under the medical direction and supervision of the attending physician, Medical Director, and Director of Nursing Services. This role emphasizes the assessment, monitoring, and implementation of physician-ordered treatments, while ensuring care is delivered in accordance with current standards of practice, regulatory requirements, and facility policies.

You will also assist in modifying the treatment regimen to meet the physical and psychosocial needs of the resident, in

accordance with established medical practices and the requirements of this state and the policies and goals of this facility.

Essential Responsibilities

Perform respiratory treatments and assessments, including but not limited to:
Nebulizer treatments
Oxygen therapy monitoring
Incentive Spirometry and Respiratory Program Assessments
Respiratory status monitoring and reporting changes

Monitor effectiveness of treatments and communicate changes in condition or lack of response to the physician and Interdisciplinary Team.
Ensure timely completion and documentation of all physician-ordered treatments.
Perform comprehensive assessments including skin, wounds, and respiratory status.
Monitor residents for adverse reactions or complications related to treatments and report appropriately.
Maintain inventory of treatment supplies, wound care products, and respiratory equipment stocked.

  • Communicate regularly with residents and their family about Plans of Care, PT, OT and other treatment protocols.

  • Excellent communication skills and strong customer service for residents. 

  • Maintain an acceptable standard of nursing practice and professional decorum.

  • Chart nurses’ notes in an informative and descriptive manner that reflect the care provided to the resident, as well as the resident’s response to the care. 

  • Provide resident care including carrying out physician’s orders for care, including providing medication and treatment. 

  • Identify, manage, and treat skin disorders and other conditions requiring nursing treatments in accordance with physician orders.

  • Perform treatments in a manner that protects the dignity and self-respect of the resident. 

  • Provide assessment services related to skin, wounds, and respiratory status, using observation, inspection, and palpation.

  • Assist in preparing and updating care plans for treatment-related issues. 

  • Assist physicians and other healthcare providers when making visits and conducting examinations. 

  • Communicate with physicians and other health professionals regarding resident care, treatment and condition. 

  • Participate in the orientation and on-going training of nursing staff. 

  • Report significant findings or changes in condition and potential concerns to RN Supervisor and /or Director of Nursing (DON). 

  • Many residents may be upset or in need of reassurances at times and will benefit from your services the most if you are patient, caring and compassionate. 

  • Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator, Director of Nursing or Assistant Director of Nursing. 

Supervisory Requirements

Assist with the overall supervision of the nursing staff. 

Qualification

Education and/or Experience

  • Possession of a Nursing Degree from an accredited college or university. 

  • Two (2) years or more experience, preferably in a long-term care facility. 

Language Skills

Ability to read technical procedures. Ability to read and comprehend policy and procedure manuals. Ability to effectively present information and respond to questions from managers and employees. 

Mathematical Skills

Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. 

Reasoning Ability

Ability to solve practical problems. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

Certificates, Licenses, Registrations

  • Maintain a current, unencumbered license as a Registered Nurse or Licensed Vocational/Practical Nurse. Current CPR certification. 

  • Knowledge and experience with PCC preferred. 

  • Must maintain all required continuing education for licensing. 

  • Must remain in good standing with the State Board of Nursing at all times.
