Senior Doc is expanding our Inpatient Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for patients with serious or chronic illness.

Position Overview:
We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management, pain control, and quality of life for individuals with serious or chronic conditions.
This position supports inpatient palliative care, community-based care, and home visits, with flexibility to practice across settings based on patient needs and program demand.
Key Responsibilities:
· Provide comprehensive palliative care consultations across inpatient, community, and home-based settings
· Develop and manage inpatient consultation emphasizing comfort, symptom control, and patient goals
· Collaborate closely with physicians, nurses, social workers, chaplains, and interdisciplinary care teams
· Serve as a clinical leader within the interdisciplinary team
· Educate patients and families on disease progression, treatment options, and goals of care
· Demonstrate strong time management while managing a diverse patient caseload
· Participate in interdisciplinary meetings, case conferences, and care planning
Qualifications:
· Active California Nurse Practitioner or Physician Assistant license
· Strong interdisciplinary team leadership skills
· Excellent time management and organizational abilities
· Comfortable with home visits and community driving
· Strong communication skills with patients, families, and care teams
· Ability to work independently while coordinating care across settings
Preferred / Plus Qualifications:
· Credentialed at either Orange County, LA County, Long Beach, Temecula Valley, Riverside, San Bernardino, San Diego County area hospitals
